Welcome to the IWMF Patient History Database. The purpose of this database is to
collect and summarize information regarding the experiences of those battling
WM. It is intended that the information collected will prove to be a valuable
resource for patients, caregivers, and the medical community to better
understand how the disease impacts the patients, and the mechanisms patients
employ to live more productive lives with the disease. If this is your first
time entering this Database tool, please be aware that it will take a few
minutes to enter the required information. Should you have to stop before
completing any particular questionnaire, rest assured that all your data has
been stored up to that point. All you have to do is remember your userid and
password that you will request below. If you are a returning user, please feel
free to log in to provide additional information and/or review information that
was previously entered (eg. Patient history or Test results). In the interest of ensuring
that your data and identity are private and secure, you will be the only person who knows your
userid and password, and medically-related information is NOT linked to any of
